Enrollment

To register for enrollment at Skyview Middle School an enrollment packet must be completed. You can request a packet by calling 651-702-8031, or stopping by the Guidance Office Monday – Friday between the hours of 8:00–3:30.

An intake meeting with the parent/guardian, the student and the grade-appropriate counselor is also required. The meeting takes approximately one hour and can be arrange when you call or stop by to request the enrollment packet.

The following information must be provided before the student can start school:

Parent/Guardian Identification

Student Transcript – At least the most current report card

Proof that Immunizations are in compliance with State law

Current IEP-for Special Education students

Birth Certificate

Proof of Residency – A lease agreement if renting, or a purchase agreement if owning. A current utilities bill is also acceptable. If you are residing temporarily with another family, we require a notarized statement from the owner or renter with the names of people who are living in the household and the anticipated length of stay. A proof of residency form, which is included in the registration packet, must also be completed.

It is the parent/guardian's responsiblity to provide the above information. Enrollment is not considered complete, and the student cannot start school until this information is received.
